The three types of interventions are effective, have low costs, and offer a good prospect of implementing a CCOJ-centered model for designing or implementing a nursing care intervention project. Description This study is an interventional study. It is a randomized controlled trial to examine, theoretically, the interplay between different intervention approaches to change in care in a non-CBO setting and clinical outcomes, including outcome scores, satisfaction ratings, and health status. Design The study protocol is approved by the Research Ethics Board and conducted in a group of 2 community-based centers (PCLBCs) across the Central Ohio University and Cleveland Clinic. Clinical features of patients seen during the study period included being on an on-/off-line intensive care unit (CAPU) or attending a primary care facility (PCF) with non-CBO home care.
