In 2010, CHN gained the recognition of a high percentage of inpatient visits to and from outreach services. The CHN program has a wide variety of goals including disease management, treatment goals, and home care. CHN programs provide a high level of competency and skills to improve the patient and patient-centered communication and health. Through public statements and calls with nurses, the CHN program is an innovative and committed team to promote the patient-centered message of CHN and provide effective patient assessment and care. CHN programs are not designed specifically to provide for a multidisciplinary goal or patient treatment, but they have a rigorous quality development plan to train nurses, provide specialist counseling, and mentor patients. CHN programs have a strong commitment to patient-centered communication, and the strong involvement of community members supports CHN programs and staff as a force in its development.
